Photo Release Waiver

This License and Waiver Agreement ("Agreement") is entered between Montbello Organizing Committee (MOC) and the persons First and Last Name listed. If Participant is under the age of 18, I certify that I am their guardian and represent his/her rights.

Participant has benefited from programs conducted and/or supported by MOC, and has agreed to allow MOC to use his or her likeness and personal story to help MOC promote its programs and/or seek financial support. This Agreement is described in additional detail below.

1. Participant gives and grants forever to MOC, and its licensees, successors and assigns (herein collectively called the "Licensed Parties"), the right to use, edit, modify, excerpt from, reproduce, distribute, publish and copyright (in any media including advertisements, editorial, digital or social media) his/her name, image, voice, picture, portrait, likeness, and history related to the participation in and receipt of benefits from programs conducted and/or supported by MOC, for the purpose of promoting the Licensed Parties' programs and/or partner organizations and residents that work and live in the Greater Montbello Community.

2. Participant agrees that, regardless of the form or media used, all pictures, portraits, presentations, likenesses, portrayals, depictions and statements or descriptions pertaining to Participant’s circumstances and experiences relating to participation in programs conducted by or services provided by MOC are owned exclusively by MOC, which may copyright material containing the same. Participant may receive copy of such materials but shall not authorize their use by anyone else without written agreement from MOC.

3. Participant agrees that no advertisement, promotion or other material utilizing Participant's name, image, voice, picture, portrait, likeness or description of circumstances and experiences relating to participation in programs conducted or supported by MOC need be submitted to Participant for approval.

4. Participant understands and agrees that he or she will not receive any money, benefit, or compensation from and releases MOC from any and all claims whatsoever in connection with the use his/her name, image, voice, picture, portrait, likeness, or history pertaining to circumstances and experiences relating to participation in programs conducted or supported by the Licensed Parties.

I HAVE CAREFULLY READ THE FOREGOING LICENSE & WAIVER AGREEMENT AND UNDERSTAND ITS CONTENTS. I’M AWARE THAT I AM WAIVING CERTAIN LEGAL RIGHTS I OTHERWISE MAY HAVE.
